	Dr. Harleen Chohan BSc, PharmD, ACPR2 Vancouver General Hospital The CSHP Foundation is pleased to award the Integrating Environmental Sustainability into Pharmacy Practice Grant in support of the PHAST Initiative (PHarmacist-led Inhaler Assessment for Sustainable Therapy). This initiative aims to implement systemic improvements in sustainable inhaler care are hindered by several key barriers through the creation of a pharmacist-led clinical decision support tool and education program at Vancouver General Hospital. Dr. Harleen Chohan, a Clinical Pharmacy Specialist in Lung Transplantation at Vancouver Coastal Health based at Vancouver General Hospital, and Clinical Assistant Professor at the University of British Columbia, will lead the project as part of her clinical role. A graduate of the University of British Columbia Entry to Practice PharmD program, Dr. Chohan has completed Year 1 and Year 2 pharmacy residency training with the Lower Mainland Pharmacy Services Program in British Columbia. Her work focuses on advancing sustainable and patient-centered pharmacy practice, with interests spanning medication optimization, health literacy, and implementation of innovative care models. Her commitment to improving patient outcomes while addressing the environmental impact of healthcare is central to this project. Grant funded by a donation from:
