CSHP Ontario Branch Awards Application Form 

Deadline for submissions: Sunday, July 14, 2024  by 5:00 pm ET

The E. Amy Eck Award

We will be accepting nominations for the two following categories:

1. A unique pharmacist-led project resulting in a significant improvement in patient care within a health organization or outpatient pharmacy practice environment.

2. A unique pharmacist-led humanitarian/ volunteer initiative within a health care community.

One award will be presented for the best overall submission.

The Ontario Branch Literary Award

We will be accepting nominations for the two following categories:

  1. An article describing original research or a new program
  2. A review or case report. 

The article submitted should be pharmacy-related (e.g. therapeutic review, case report, program established, and original research) and must have been published or accepted for publication in a peer reviewed journal in the year prior to the deadline date.

The Douglas J. Stewart Award

This award recognizes excellence in hospital pharmacy administration. The applicant must have demonstrated exemplary skills in implementing a NEW clinical or drug distribution program to the hospital. The program must already be implemented.

The Bill Wilson Patient Safety Award

This award recognizes an applicant who has demonstrated exemplary skills in implementing a NEW patient safety initiative at the direct patient care facility.

The Innovative Information Technology Award

This award recognizes an applicant who has demonstrated exemplary skills in developing and implementing an innovative locally-derived information technology solution (e.g. application, electronic tool or program) to improve patient care or pharmacy practice at the hospital.

Award for Primary Care Initiative

This award recognizes a unique pharmacist-led initiative resulting in a significant improvement in patient care within a collaborative primary care setting (e.g. Family Health Team, Community Health Centre, Long Term Care Facility, etc.)
